Point to Point's staff understands the overwhelming anxiety family members may feel when confronted with a relative's unexpected medical setback regardless of age.
Additional challenges occur when elderly or disabled relatives are long distances away and the responsible son, daughter, niece, or nephew is unsure how to relocate their loved one closer or even if bringing them home is possible.
Many clients we transport are currently residing in assisted living homes or skilled nursing facilities and require assistance with bed to wheelchair transfers, dressing and toileting.
After carefully reviewing medical records and consulting with the attending physician, our staff develops a travel plan tailored for the individual's physical, mental and medical needs.
Based on your relative's mobility, a medically trained travel escort is selected from our staff of nurses and practitioners of physical or occupational therapy.
Point to Point Travelers makes all travel arrangements, secures medical equipment and arranges oxygen use if required for the trip.
A bowel and bladder program is implemented for incontinent clients to help put them at ease during the trip.
Family members having loved ones diagnosed with Alzheimer or dementia
are encouraged to provide stories of the client's lighter side to help
our staff deliver the least disruptive journey.
During transport, our trained staff will monitor your family member's vital signs, employ a DVT (Deep Vein Thrombosis) prevention program, encourage fluids to prevent dehydration and comply with any diet restrictions.
Family members are welcome to travel with us but are not required.
Our staff will pack all personal belongings as well as ensure your loved one is appropriately dressed for the trip.
Upon arrival at the new facility or home, our escort unpacks your relative's belongings, debriefs receiving staff of the trip, and delivers any personal insight that may help with the client's transition.
In some cases our escort has extended their stay in the new area to assist setting up housing needs, light shopping, and orientation to community resources.
To ensure a successful transfer, we work intently with family members and medical staff at each facility. Every client's review addresses a broad range of mental impediments and behavioral cues including preventative and recovery strategies for mental fits. With a compassionate approach during planning, a thorough understanding of behavioral triggers, and experienced staff, we avoid using medications that may impede an already medically or emotionally fragile traveler.
Orthopedic Precautions: We review every patient's orthopedic precautions to assure our escorts fully understand the transfer challenges and mobility limitations which occur during movement between facilities. Individuals that require orthopedic precautions are typically recovering from hip, knee, or back surgery.
Neurological Disorders: Clients with neurological disorders are carefully evaluated to best prepare our escorts for physical and cognitive deficits associated with strokes, Parkinson and spinal cord injuries. Normally, clients ready for a transfer have completed a rehab program and are moving on to specialized facilities or returning to family.
Cognitive Impairment: Point to Point Travelers has moved many clients in various stages of Alzheimer's or other types of dementia, and patients recovering from traumatic brain injuries that have long term cognitive effects. Long and short term memory loss, confusion, limited awareness of potential hazards during ADLs and other cognitive challenges require a blend of skills to anticipate and redirect the patient away from negative behaviors or outbursts.
Our staff uses experience, creativity, and specialized equipment to safely transport clients ranging from non-ambulatory to those using adaptive walking devices.
Due to flight restrictions, clients must have the ability to sit up for take off and landing. Once airborne, clients can recline in first class comfort for the duration of the flight.
We provide the appropriate amount of assistance with wheelchair transfers, toileting, and feeding techniques to increase confidence and hopefully preserve self esteem.
A journey with our proactive escorts is designed to reunite family as well as strengthen the client's self confidence and continue building on the recommended rehab program. During the trip, our travel companion helps the client apply skills recovered during therapy.
If a client is on oxygen or has a compromised respiratory system, our staff makes all arrangements for oxygen consumption during flights and on the ground. During transport, our escort monitors the client's oxygen saturation level and other vitals to ensure patient stability and comfort.
If clients require wheelchair use during transfer, Point to Point Travelers makes arrangements for any equipment used during the trip. This effort may decrease a facility's medical equipment loss.
If you are discharging a resident from your facility, our staff will pack all personal belongings and medical supplies required for the trip. At the receiving end, we provide a thorough trip debriefing to medical staff.
